4. Complete & submit this Application & $55 Processing Fee or $525 Deposit (Covers Processing Fee & Deposit to secure your spot) (non-refundable). Space is Limited. ✅

6. We will review your application & get back to you. ✅
7. If you fit our participant/Practitioner criteria, We will contact you for a follow up Interview & answer any questions you may have. If you are not a fit, we will happily return $450 of your Deposit. ✅
8. Once Accepted (Yay!), you can Register & Pay for your program. We will then send you a Welcome Letter with additional information to help you prepare for your program. ✅
9. One of the ways you can best prepare and get familiar with what you will be learning is to get 1:1 Session work with 1 of our IHP Practitioners-In-Training. This will give you a hands-on feel for how the work works, what you would be doing as a Practitioner, and helps to clear imprints Before you come into the program. Game Changer! ✅

Participation in this program is by invitation, nomination, application, and selection only at this time.

Studying & training with Master Teacher, Diana H Dokos, and being part of the Practitioner cohort is a rare and life-changing opportunity. For that reason, we screen for highly dedicated & talented students who are likely to Rock this Opportunity, complete their program in full, and make a positive difference in the lives of others & the planet with what they learn.

The "Conscious Creators" program is Fun! Super Fun! Our students often refer to it as "Shaman Camp". 🤓😇 And... It's also Jedi training. So we want to be sure you're ready, motivated, prepared to dive in and make the most of the Opportunity. We expect that Practitioners-in-Training and interested applicants are highly dedicated, have done a fair amount of deep personal work, are prepared to be Practitioners not just clients, are highly ethical, and have made the appropriate arrangements in their life, finances, and work schedules to make their training, mentorship, and client practicum a priority.

NARRATIVE QUESTIONS
1. What is inspiring or motivating you at this time to participate in the "Conscious Creators" program and/or be an Integrative Healing/Shamanic Practitioner? What is the most valuable & motivating aspect of this opportunity for you?*
2. The Intention of the "Conscious Creators" program is to move people, projects, communities, and creators forward... consciously, creatively, and in flow. Who and/or What do you most hope to move forward with what you learn? What are your Intentions for the "Team UP" program?*
3. Are you affiliated with an organization, business, or community in which you would be using what you learn? Name of the organization, business, or community and your position in it. What does this group do? How would your participation in our program contribute to the well-being of this organization, humanity, the planet?*

5. Right Relationship & Conscious Engagement: One of the central teachings & foundations of our work are the principles and practices of Right Relationship & Conscious Engagement. These principles & practices support living, leading, and serving from a place of greater grace, ease, harmony, thriving, prosperity, and flow as a way of life. There are many ways to come into Right Relationship (alignment, integrity, harmony) and Conscious Engagement (to engage consciously with care and consideration for self AND others, regardless of what presents itself.) What work have you done in your own life/profession/relationships in this regard? Are you desiring to learn more about Right Relation & Conscious Engagement?*
6. Struggle & Drama Dynamics: A second foundational teaching, principle, and practice we teach and commitment we make as students, teachers & Practitioners is to be aware of and steer clear of Struggle & Drama Dynamics (victim, rescuer, bully, avoidant) and victim consciousness (blame of others, seeing things as happening "To Me", projection). We do this early on. What work have you done to address this internally and in your life up to this point? Are there teachings and/or practices you find most helpful in this regard?*
7. Imprints: A third main teaching and shamanic practice you'll learn about in both Part 1: Foundations and Part 2A: Integrative Healing Practitioner training is about clearing and addressing "Imprints". Imprints are patterns in the cells, chakras, consciousness & energy field that are often the root cause of repeating patterns, struggle, drama, stuckness, loss, health issues, financial and other difficulties. When one is in a new situation, or experiencing stress, uncertainty, disagreement with another, or feeling fearful or "wronged" in some way, this can be a symptom of imprints and/or can activate imprints. How do you typically respond in such scenarios? What are you committed to do should such imprints or scenarios arise during your training or mentorship?
8. As part of your Levels of Self-Mastery (aka "Jedi training" & Up-Leveling), you may experience sudden & unexpected Opportunities (personally, professionally, spiritually, financially, etc). You may also get "tested" along the way via compelling distractions, activated imprints, resistance, upper limits, and the like. This is often a natural part of up-leveling, the shamanic journey, and lived "initiation" into truly living & relating from the Authentic/Higher Self. While healing is certainly part of our program, We are primarily interested in training students to be Conscious Creators and Practitioners who assist their friends, family, clients, communities, and businesses to successfully navigate these aspects of life.
